To assist hospitals in improving care transitions and avoiding preventable readmissions, DVHC has partnered with The Health Care Improvement Foundation (HCIF) through their Preventing Avoidable Episodes (PAVE) Project to develop a reference guide—the PAVE Payor Passport—to facilitate communications between hospitals and health plans when patients are being admitted and/or discharged.

The Payor Passport is designed to assist hospital utilization management, emergency department, and discharge planning staff in connecting patients with the information, medication and equipment, and post-acute care services needed to avoid preventable readmissions.
